Reminder: An updated health history form is required each year. If you receive an email to fill out a new form please complete it before your appointment, or plan to arrive 10 minutes early to fill it out at the clinic.
Please note if your appointment is booked with Damilola Ogundipe RMT, she is a fully registered Massage Therapist; however, she is not yet set up for direct billing to Bluecross. Payment will be required upfront if your provider is bluecross.
New Clients – Allow a Little Extra Time
During your first visit, we’ll go over your health history and assessment as part of your appointment. To make sure you get the full treatment and the best care you deserve, consider booking a bit longer — for example, 75 minutes if you want a full 60-minute hands-on session.

Payment is due on the day of treatment. Bring your insurance card and a backup payment method. We only can direct bill to ONE insurance company per client.
